The simple definition

An AI medical scribe is software that turns a clinical encounter into structured documentation. A human scribe sits in the room and writes the note; an AI scribe does the same job from the audio, drafting a note the clinician reviews and signs.

Ambient vs dictation

There are two broad styles:

  • Dictation is the clinician speaking to the system directly (“start note, chief complaint is…”).
  • Ambient scribing is the system listening to the natural conversation between clinician and patient and generating the note from it, no dictation required.

Ambient is where the field is heading, because it removes the extra step entirely. It is also the harder engineering problem.

How it works

Look for clinical-grade speech handling, grounded output with per-field confidence, clean integration into the EMR you already use, and a clear human-in-the-loop review step. If a vendor cannot explain how it handles uncertainty, be cautious.
